Base Salary: $95,000–$110,000/year
Sign-On Bonus: $3,000 Metro is looking for a motivated and passionate full-time Physical Therapist to provide in-home care throughout Eastern Suffolk County.

Responsibilities
- Accept and treat patients within your designated geographic area
- Evaluate patients’ needs for physical therapy in the home setting and develop individualized treatment plans
- Develop a plan of care and treatment goals in consultation with the physician, patient, and family
- Plan, prepare, and implement customized therapy programs to restore function and promote independence
- Instruct family members, PTAs, Home Health Aides, and caregivers on appropriate therapy techniques
- Provide timely and accurate documentation for each visit
- Communicate effectively with patients, families, and interdisciplinary team members, especially around discharge planning
- Deliver care in compliance with agency policies and professional practice standards
- Graduate of an accredited Physical Therapy program
- Licensed Physical Therapist in the state of New York or eligible for licensure
- Home health experience is preferred but not required — we will provide you with a mentor to help with documentation and compliance
- Ability to travel to patients’ homes within the assigned geographic territory
The anticipated base salary range for this position is $95,000–$110,000 annually. Salary is based on various factors, including relevant experience, knowledge, skills, other job-related qualifications, and geography.
This position is eligible for a $3,000 sign-on bonus. Additional terms and conditions may apply.
